The pouch is a polyester mesh which breathes well. Quality seems very good. The pouch doesn't seem to pull the boys down between my legs, so it's comfortable. Note: I have a 33" waist and usually get a Large in jockstraps bec I don't like a supertight fit. In this jock, however, I could easily have used a Medium and still not feel so constricted. So I would say the sizing is very accurate to standard US measurements -- upsizing is not required.
